The default prompt is "repl> 〉", with the ASCII characters in dark green and bold. This is inconsistent with the prompts created by default by with_name(), with_prompt(), and with_formatted_prompt(). Use "repl〉" instead. Also apply some cleanups to the prompt docs.
Modify the DefaultPrompt directly.

While here, apply some cleanups to the prompting code (see individual commits). If those aren't wanted, let me know and I can remove some or all of them.
