How to handle missing time sig on first measure.

Unless I missed it, the spec is silent about what it means if the first measure lacks a time signature. Ideas that occur to me are

• it is an error in the file
• defaults to 4/4 (not crazy about this)
• must be inferred from all the part measure sequences. (I like this even less.)

I would like to treat is as an error in the file, but I wish the spec said so. FWIW, this seems a little different than key signature. It makes sense for a file to default to having no sharps or flats, but I don't think any default make sense for time signature.

We haven't fully discussed Senza-Mensura or other non-measured musical spaces -- that is at least a 3rd option.

Note that a default of 4-quarters is possible without 4/4 -- it could be 2/2, 8/8, etc. -- that is another possibility.
