Custom GitLab URL (similar to github enterprise)

The feature request

Add support for self-hosted or enterprise GitLab instances.

Proposed solution

Add the possibility to add the URL of the GitLab instance, similar to the way the GitHub Enterprise is handled.

Additional context

There are lots of self-hosted instances of GitLab, for example, gitlab.gnome.org, and support for this feature should be pretty straightforward.

I'll see if this is doable. It will require a big refactor of the way accounts are currently handled. If the size of the diff ends up being too large, I might reject this feature, because I want to keep the merge conflicts reasonable when pulling from upstream.

The upside is that if I manage to make it work, that will probably also solve #2 for free.

Can you add Gitlab.com to the Clone context window like Github.com

Blocked by #2. Once the big refactor that is ongoing at #41 is reviewed and merged, we will be able to start working on custom endpoints for Bitbucket and GitLab, as well as self-hosted options (as requested here: #12 (comment))
