chore: remove self-managed AI artifacts and wiki/sandboxes submodules (#924)
Decommission orchestra as a self-managed repo — tooling is now managed
from the orchestrator. Removes .claude/, .codex/, .ralph/, the wiki and
sandboxes submodules (and the exec_server service that built from
./sandboxes/ubuntu), plus the now-dangling Makefile / .gitignore /
.dockerignore and CI path-filter references.
