GPU: Integrate VK_KHR_device_fault

This [extension](https://docs.vulkan.org/refpages/latest/refpages/source/VK_KHR_device_fault.html) falls under the "optional but _extremely_ nice" umbrella. In addition to giving better crash data for supported devices it also lights a fire under us to do a better job about checking for `VK_ERROR_DEVICE_LOST` which would be the main reason to call these new functions.
