Describe the bug
Latest Action tracking status is not reflected/displayed correctly on action planning

To Reproduce
Steps to reproduce the behavior:

1. On the tracking tab, click on one of the plus icons to add an action status
2. Fill in the popped up form
3. Click save

Expected behavior
On clicking save, the popup should disappear, the page should reload and the new added action status should be seen. If the page is reloaded the new action status should still be available
